3．妊娠中の広汎性子宮頸部摘出術

文献概要

●近年，若年者の子宮頸癌発症率が増加していること，女性の晩婚化，晩産化が進んでいることより，妊娠して産婦人科を受診し，子宮頸癌を指摘されるケースが増えてきている．

●患者や家族が強い妊娠継続を希望する場合，①胎児が子宮外で生存可能になるまで治療を待機する方法，②プラチナ製剤を基本としたneoadjuvant chemotherapy（NAC）を先行し，胎外生存が可能な時期になってから帝王切開術で児を娩出後に子宮頸癌に対する根治術を行う方法，③根治を目指す目的で妊娠中に広汎子宮頸部摘出術を施行する方法が報告されている．

●妊娠中の腹式広汎子宮頸部摘出術はこれまで少なくとも14例試みられ，妊娠15週以降に施行した9例中8例に生児が得られている．
