既往早産妊婦の管理─②プロゲステロン療法

●早産の既往はきわめて高い早産リスクで，既往早産の時期が早ければ早いほど，相対危険率は増加する．

●早産既往妊婦の管理としては，本邦でも保険が適用される17-OHPCが第一選択の予防治療になる．

●妊娠16週以降，週1回，17-OHPCを250 mg筋肉内注射する．

●頸管短縮を伴う場合や切迫早産徴候がある場合は，17-OHPCに加え，状況に応じた対応が必要になる．
