婦人科癌リンパ節転移の術中迅速遺伝子診断

はじめに

 分子生物学的な手法を用いた遺伝子診断は,腫瘍の病理診断,分子標的治療の適否の判定など癌の診療には欠かせないものになっている.リンパ節転移診断においても,乳癌などでは術中迅速診断にも応用されるようになった1).術中迅速遺伝子診断によるリンパ節転移の検出は,センチネルリンパ節(SN)を同定し,その転移の有無により治療方針(系統的リンパ節郭清施行の有無)を決定する際に有用な情報になる.婦人科癌においてもSN同定の妥当性が検証され,リンパ節転移の発見率の上昇などその有用性が明らかにされてきている2〜5)が,術中迅速診断については凍結標本一切片の病理学的診断では術後のultrastagingにより見逃しが発見され6, 7),この結果で治療方針を決定する難しさも指摘されている.そのため,微小な転移も見逃さないような精度を有し,さらに多数の切片を検索するような労力を要することのない容易な迅速診断の確立が望まれている.

 本稿では当科において検討しているone-step nucleic acid amplification(OSNA®法)による子宮頸癌,子宮体癌におけるリンパ節転移の術中迅速遺伝子診断の可能性と今後の展望を中心に概説する.
