頸管縫縮術の適応と有効性

Point
◉頸管縫縮術の目的は流産・早産の防止であり，少なくとも児の未熟性による合併症が少なくなる時期まで妊娠を延長することである．頸管縫縮術を行ったことによって，かえって破水を生じたり，抑制困難な子宮収縮が生じることは避けなければならない．
◉適応については，手術のリスクも含め，保存的管理（縫縮術を実施しない場合）の予後とよく比較し，症例ごとに十分に検討する必要がある．
◉習慣流産の症例に対して実施する頸管縫縮術の予後に関して，頸管縫縮術の実施が有用であるという明らかなエビデンスは示されておらず，現時点では個々の症例に応じて対応している．
◉慎重な妊娠管理にもかかわらず，妊娠中期に超音波上の頸管長短縮や胎胞を視認することがある．このような症例に対する治療的頸管縫縮術は，症例によっては有用なことがある．
