婦人科腫瘍におけるセンチネルリンパ節生検の可能性

はじめに

　センチネルリンパ節（SLN）は，腫瘍が原発巣から最初に転移するリンパ節を指す．SLNを同定し，転移がないことを証明できれば，他へのリンパ節転移は成立していないはずであり，系統的リンパ節郭清を省略することができるという概念である．SLN生検により系統的リンパ節郭清による出血，神経損傷，リンパ囊胞，リンパ浮腫などの合併症発症を避けることができ，悪性黒色腫や乳がんでは確立している．婦人科領域，特に子宮頸がん，子宮体がんにおいては，臨床研究段階である．

　本稿では子宮頸がん，子宮体がんと外陰がんにおけるSLN生検の可能性について概説する．
