当院で加療した妊娠中の付属器腫瘍の茎捻転13例の検討

▶要約

　妊娠中の急性腹症の鑑別は多岐にわたり，診断に苦慮することがある．今回われわれは，妊娠中の付属器腫瘍の茎捻転13例について，その特徴と手術様式による差異を，同時期に当院で管理した非妊娠時発症の付属器腫瘍の茎捻転症例64例と比較検討した．妊娠時の付属器腫瘍の茎捻転例では，非妊娠時に比較して小さな付属器腫瘍や，妊婦健診で指摘されていなかった付属器腫瘍の茎捻転症例があった．

　診断には超音波だけでなくMRIが有用であり，付属器腫瘍の茎捻転が否定できない場合には十分に検索することが必要である．手術では再発を予防するために捻転の解除だけでなく，囊腫の摘出や囊腫の穿刺が必要となる．8例に対して行った腹腔鏡下手術では，合併症，周産期予後において問題なく施行できた．妊娠中に付属器腫瘍の茎捻転を疑う場合にも，妊娠子宮の大きさに注意をすれば，腹腔鏡下手術による低侵襲な診断と治療は選択肢になると考える．
