Edwardsiella tardaによる子宮付属器膿瘍からの敗血症性ショックをきたした1例

　Edwardsiella tardaは爬虫類などの腸内常在菌として存在するグラム陰性桿菌である．人への感染は稀であるといわれるが，胃腸炎患者の糞便から検出されることがある．基礎疾患のあるcompromised hostへの感染や海外渡航歴，魚介類の生食などで感染するといわれるが，その感染経路に関しては不明な点も多い．今回，基礎疾患のない49歳女性のEdwardsiella tardaによる子宮付属器膿瘍から敗血症性ショックに至った症例を経験した．子宮付属器摘出術を施行し，抗菌薬投与と血液濾過透析，エンドトキシン吸着療法などを施行し改善に至った．本菌による骨盤内感染の報告は国内でも散見されるが，基礎疾患がなく海外渡航歴や魚介類の生食などの明らかな感染経路がない報告は稀である．敗血症に至った場合の致死率の高さより，婦人科領域での感染例では本菌も念頭に置いて治療をする必要があると考えられた．
