症例

患者　36歳，初産婦，自然妊娠．

既往歴・家族歴　特記すべきことなし．

現病歴　妊娠初期に一絨毛膜二羊膜双胎と診断され，定期的に外来管理されていた．妊娠20週4日の超音波検査所見を示す（図1，2）．

A児 : 推定体重425g（＋1.2SD）．最大羊水深度（maximum vertical pocket : MVP）6.2cm（図1a）．臍帯動脈および静脈管血流波形は順行性．膀胱は描出可能．胎児水腫を認めず．

B児 : 推定体重205g（−2.6SD）．MVP 0.3cm（図1b）．臍帯動脈血流波形を図2に示す．静脈管血流波形は順行性．膀胱は描出可能．胎児水腫を認めず．
