卵巣成熟囊胞性奇形腫の再発に関する検討

　卵巣成熟囊胞性奇形腫（mature cystic teratoma : MCT）は，全卵巣腫瘍の約15〜20％と比較的多く発生する良性の胚細胞性腫瘍である．腫瘍が大きくなれば手術療法が推奨されるが，術後の再発率や再発のリスク因子については報告が少ない．今回，当科で過去10年間にMCTに対して卵巣温存手術を行った230例を対象に，再発のリスク因子について後ろ向きに検討した．再発を認めたのは18例であり，全体の再発率は7.8％（18/230），5年累積再発率は18.6％であった．再発までの観察期間中央値は55か月（3〜192か月）で，術後5年以上を経て再発を認めた症例が61％（11/18）であった．累積再発率におけるリスク因子の検討では，年齢30歳未満（ハザード比2.62，95％信頼区間1.34─5.67）と茎捻転・破裂あり（ハザード比2.19，95％信頼区間1.09─4.16）の2つの因子が抽出された．30歳未満の若年者や術中に茎捻転・破裂の所見を認めた場合は，再発を念頭において年単位の長期フォローアップが必要である．
