子宮頸部原発と考えられた明細胞腺癌の1例

▶要約

　子宮頸部明細胞腺癌は，子宮頸癌の0.4％を占める非常に稀な疾患である．今回，子宮峡部を占拠し，原発部位の診断に苦慮した子宮頸部明細胞腺癌の1例を経験したので報告する．症例は36歳，1経妊0経産，2か月前からの不正性器出血を主訴に受診．子宮腟部の所見は正常であったが，子宮頸管内の細胞診と組織診から明細胞腺癌の診断が得られた．骨盤MRIでは子宮頸部から峡部を占拠する5×3cm大の腫瘍を認めた．原発部位が最大の問題点となったが，内診や画像などの臨床所見と，免疫組織学的検討を加えた病理所見から，子宮頸部明細胞腺癌Ⅰb2期の診断に至り，手術療法と術後追加治療を行った．子宮頸部明細胞腺癌は，極端な内向性発育の性格をもち，扁平上皮癌や他の腺癌と比べ発見が遅れやすく，子宮体癌との区別が必要なケースもあるなど，色々と問題点が多いとされている．子宮頸部明細胞腺癌について文献的な考察を加え報告する．
