精索静脈瘤（臨床・手術）の適応と限界

●精索静脈瘤は男性不妊患者の約3割を占めており手術治療が有効である．最も行われている術式は顕微鏡下の低位結紮術である．

●手術後3〜6か月で精液所見は改善する（わが国の調査では患者の70％程度）．海外のRCTでも同様の結果が得られている．

●近年の報告で精索静脈瘤手術により精子のDNA断片化も改善することがわかってきており，ARTを行うカップルに対しても有効である可能性が示唆されている．
