一過性大腿骨頭萎縮症により大腿骨頸部骨折を生じた双胎妊娠の1例

　妊娠30週に足関節および股関節の疼痛を主訴に発症し，妊娠36週で大腿骨頸部骨折を生じ，一過性大腿骨頭萎縮症と診断した双胎妊娠の症例を経験したので報告する．

　症例は35歳1回経産婦で，双胎妊娠であった．妊娠30週から右足関節痛が出現，31週ごろから右股関節痛も加わり，33週ごろからは杖歩行となっていた．近医整形外科を受診するも妊娠を理由にX線検査を見送られ，経過観察を指示されていた．妊娠36週0日に自宅で転倒後歩行困難となったため救急搬送され，骨盤部X線写真で右大腿骨頸部骨折と診断した．妊娠37週0日予定帝王切開実施後，術後6日目にTHA（全人工股関節置換術）を行った．術後12日目に退院となり，術後2か月半で右股関節運動痛は消失，4か月で日常生活動作が正常化した．妊婦のTOHは稀ではあるが，妊娠後期の妊婦の訴えとして経験する可能性があり，鑑別疾患として考慮し，適切に紹介する必要があると考えられた．
