重症妊娠悪阻からWernicke脳症とRefeeding症候群を発症した1例

　Wernicke脳症はビタミンB₁欠乏による代謝性脳症である．今回，重症妊娠悪阻から発症したWernicke脳症およびRefeeding症候群の1例を経験したので報告する．

　症例は初産婦，妊娠7週より嘔気・嘔吐が持続，妊娠9週より経口摂取不可能な状態が持続し，3か月で12kgの体重減少を認めた．妊娠19週より全身倦怠感が増悪し，子宮内胎児死亡を確認した．また同日ブドウ糖輸液で点滴加療したところ意識障害を認め，高血糖563mg/dLを認めたことから，糖尿病性ケトアシドーシスを疑い当院に搬送となった．高血糖については精査の結果，Refeeding症候群と診断し，生理食塩水投与のみで改善を認めた．また頭部MRIでWernicke脳症と診断し，ビタミンB₁大量療法により神経症状は速やかに改善した．重症妊娠悪阻から発症したWernicke脳症の治療にはビタミンB₁の大量投与が必要であり，また経口摂取できない状態が長期間続いた場合に，急激なブドウ糖補充を行うとRefeeding症候群となるため注意する必要がある．
