子宮内膜症：手術に際しての癒着の評価のポイントは？─想定外の癒着による手術困難を避けるために

文献概要

●子宮内膜症は頻度の高い疾患であるため，あらゆる女性において子宮内膜症の存在を考慮しておく必要があり，術前に癒着の有無，部位を評価しておくことは安全に手術を遂行するために重要である．

●画像検査において，子宮内膜症の発生部位を中心とした臓器の固着や引き攣れ，組織の線維化などの所見を探すことで癒着の存在を推測できる．

●すべての癒着を画像診断することは不可能である．産婦人科臨床医として，画像検査のみに頼らず，詳細な病歴聴取と内診・双合診と併せて評価することを常に心がける必要がある．
