帝王切開瘢痕部妊娠に対しMTXを先行投与したうえで内視鏡下除去術を行い子宮を温存しえた1例

　帝王切開瘢痕部妊娠（CSP）は子宮破裂や大量出血の原因となり，時に致死的となる重篤な異所性妊娠である．近年の帝王切開率の上昇を反映し報告数も増加しつつある．経腟的に除去を試みると大量出血のリスクがあり，子宮摘出が必要になることも多いとされる．近年，子宮温存目的にメトトレキサート（MTX）を試みる報告も増えてきた．今回われわれは妊娠初期にCSPと診断し，MTXの全身および局所投与を先行したうえで，腹腔鏡観察下に子宮鏡下胎囊除去術を行い，子宮を温存しえた症例を経験したので，文献的考察も加えて報告する．
