―入院患者の急変―発熱

当直医へのコール
◉入院患者の発熱でコールされるときはさまざまだが，例えば以下のようなコールが想定される．

「術後の患者で帰室後は微熱だったが，徐々に上昇し，39℃台に上昇して来た．指示は解熱鎮痛薬投与で経過観察となっているが，それだけでよいか？」

「抗がん剤治療目的で入院中で，今回は単剤投与のみでもう終了している．明日退院予定だが，徐々に熱が上昇してきている」

「急性腹症で精査の結果，付属器腫瘍が見つかって，産婦人科に入院となった．痛み止めだけで対応するように指示が出ていたが，徐々に発熱も認め始めた」
◉コールの内容で特に注意すべき点は，以下の4点である．不十分であれば，実際にベッドサイドに行き，確認すべきである（「すぐに実施すべき処置」参照）．

・バイタルサイン

・随伴症状の有無

・患者背景（入院理由）の把握

・発熱のタイミング
