妊婦に対する子宮頸部円錐切除術の適応

●妊娠中の子宮頸部円錐切除術は，細胞診と組織診，コルポスコピーによる丁寧な観察を行い，その適応を十分に検討する．過剰な切除は避け，必要最小限のcone heightとする“coin biopsy”を心がける．

●早産率低下を目的とした円錐切除術後妊婦に対する予防的子宮頸管縫縮術の有効性を示したエビデンスはなく，個々の症例で慎重に検討する．縫縮術の有無にかかわらず，円錐切除術後妊婦は早産のハイリスク群と認識し管理する．
