双胎一児死亡における生存児の予後とその管理

●2絨毛膜双胎の場合，一児死亡が起きても生存児への影響は少ないと考えられるため，定期的にbiophysical profileを行いながら生存児のwell-beingを確認し，念のため母体の凝固異常の有無を確認しながら待機的管理を行うことが推奨される．

●1絨毛膜双胎の場合，一児死亡による生存児への影響は2絨毛膜双胎より大きいが，早期娩出が児の予後を改善するとはいえない．よって定期的にbiophysical profileを行いながら生存児のwell-beingを確認し待機的管理を行うことが推奨される．

●一児死亡後の生存児の評価としてbiophysical profileに加えMCA-PSV測定，場合によっては胎児MRIなどを組み合わせて行い，娩出時期に関しては未熟性を十分に考慮する必要があり，個別対応が検討される．
