神経内分泌腫瘍成分を伴う子宮頸部癌肉腫の1例

　子宮頸部癌肉腫は稀な疾患であり，腫瘍を構成する癌腫は扁平上皮癌もしくは類内膜癌が多く，神経内分泌腫瘍成分を伴う症例はきわめて少ない．今回，神経内分泌腫瘍成分を伴う癌腫成分と肉腫成分で構成される子宮頸部癌肉腫の症例を経験したので，文献的考察を加えて報告する．

　症例は58歳，2妊2産．帯下異常を主訴に受診し，子宮頸部に乳頭状に発育する腫瘤を認めた．子宮頸部組織診で低分化腺癌の結果であった．MRI画像で子宮頸部に48mmの腫瘤を認め，子宮頸癌ⅠB2期の診断でNACとして動注化学療法（Balloon occuluded arterial infusion : BOAI）を選択した．1コース終了後に腫瘤は縮小傾向を認めたが，副作用である末梢神経の痺れが強く，広汎子宮全摘術と両側付属器摘出術を行った．病理組織診では核腫大を伴う紡錘型細胞の増殖（肉腫成分）と，充実性包巣を形成する核異型の強い腫瘍細胞の増殖（癌腫成分）が混在していた．免疫染色では肉腫成分はp16，vimentin，PASが陽性で，AE1/AE3，CK7，CK20は陰性であった．癌腫成分ではAE1/AE3，CD56，synaptophysin，chromograninAが陽性であった．最終病理診断はCervical carcinosarcoma with neuroendcrine diffentiation，ypT1b1 N1 M0であり，術後補助療法として同時化学放射線療法を行うも，治療終了後5か月で副腎に再発を認め，TC療法を3コース施行した．CTで副腎病変は縮小を認めず，現在化学療法をCDDP＋CPT-11へ変更としている．
