維持化学療法の位置づけは？

文献概要

●ベバシズマブは，進行卵巣がんにおける初回治療時，あるいは再発時の化学療法との併用および維持療法として考慮される．

●オラパリブは，BRCA遺伝子変異が確認された進行卵巣がんにおける初回治療時の維持療法，あるいはプラチナ感受性再発で化学療法が奏効したのちの維持療法として考慮される．

●今後，オラパリブ以外のPARP阻害薬（ニラパリブ，ルカパリブ，ベリパリブ）の保険収載が期待される．投与症例の抽出方法（BRCA遺伝子変異の有無，HRDの有無など）やベバシズマブとの併用が可能になるかなど，エビデンスを注視していく必要がある．維持療法の重要性はますます高まっている．
