不妊症治療のなかで子宮内膜症はどのように扱えばよいのか？

文献概要

●卵巣チョコレート囊胞がある場合，疼痛が強いときや自然妊娠を強く望むときには保存手術を考慮する．自然妊娠の成立には，子宮内膜症病変が卵管機能に影響を与えているかどうかの判断が重要である．

●囊胞摘出術により卵巣予備能は低下するが，片側の場合は体外受精による妊娠率にさほど悪影響を与えない．

●加齢とともに卵巣予備能は低下することから，ARTへの移行時期を逸することのないように配慮する．

●重症子宮内膜症患者では，ARTにおける獲得卵子数が少なく，生児獲得率は低い．また，流産率が高い．
