子宮頸部・内膜細胞診異常のみで早期卵管癌を疑った1例からの文献的レビュー

　卵管癌は稀な疾患で，早期発見は困難なことが多い．子宮頸部・内膜細胞診で腺癌細胞を認める以外に卵管癌を疑う所見はなく，手術にて卵管癌の診断に至った症例を経験したので報告する．症例は60歳2妊2産，症状や内診では異常を認めず，経腟超音波検査では両側付属器腫大や腹水貯留はなかった．MRI，FDG PET/CTでも有意所見はなかった．内膜細胞診で清明な背景に腺癌細胞を認めたため，卵管癌および腹膜癌を疑い，腹式単純子宮全摘出術および両側付属器摘出術，大網切除術を施行した．腹腔内および摘出標本に肉眼的病巣は認めなかったが，腹水細胞診は陽性であった．病理組織診で左卵管采より高異型度漿液性癌を認めたため，最終診断は卵管癌ⅠC3期であった．術後TC療法を6コース施行し，現在術後18か月で再発はない．本症例のように，子宮内膜細胞診で正常な内膜細胞や，清明な背景に明らかな腺癌細胞を認める場合，卵管癌を念頭に置いた対応が必要である．
