計画分娩にあたっての頸管熟化の評価

●分娩誘発前の頸管熟化がよいほど分娩誘発成功率は高く，不良なほど帝王切開のリスクが高い．計画分娩での経腟分娩成功のカギは頸管熟化度である．

●頸管熟化の評価にはBishop scoreを用いることが一般的である．普遍的な基準値は存在しないが，6点以下を不良とすることが多く，3点以下は特に不良と考えられる．

●熟化不良症例に分娩誘発を行う場合は子宮収縮薬の前に頸管熟化処置を行う．3点以下の場合は，成功の可能性が低いため延期または中止（自然陣痛発来の待機）を考慮する．
