MSI検査とはどのような検査か，何がわかるか

文献概要

●免疫チェックポイント阻害薬ペムブロリズマブのコンパニオン診断としてMSI検査が臨床導入された．

●MSI-High固形がんでは腫瘍特異抗原（ネオアンチゲン）が多く発現し，T細胞の認識を受けやすくなるため，ペムブロリズマブが奏効すると考えられている．

●MSI-Highが示すことは，あくまでもミスマッチ修復機能欠損（結果）であり，後天的異常かLynch症候群であるか（原因）を確定することはできない．

●MSI検査によりLynch症候群の患者を拾い上げる可能性があり，MSI検査の際は遺伝学的な背景を含めた理解と患者・家族への十分な配慮が必要である．
