hCG投与とそれ以外のLHサージ誘発法―double/dual trigger・r-hCGとその臨床的意義

▶ARTにおけるLHサージ誘起法には，現在，GnRH―アゴニストの点鼻法とhCG製剤の皮下/筋肉投与があり，hCG製剤は半減期が長く，OHSSのリスクを伴う場合は，GnRH―アゴニストの点鼻法が望ましい．
▶GnRH―アゴニストは即効性があり，自然のLHサージと似た状態を作ることができる．一方，下垂体の脱感後やGnRH―アンタゴニスト使用直後は，hCG製剤を用いて誘起を行う．
▶高ゴナドトロピン状態からの卵巣刺激の際には，hCG製剤を使用した誘起が望ましい．
