帝王切開瘢痕部の瘻孔により生じた膀胱子宮窩腹膜下の血液貯留が原因と考えられた難治性頻尿の1例

　帝王切開術後には縫合部に癒合不全による陥凹や菲薄化が生じることがある．今回，帝王切開後の子宮瘢痕部に生じた瘻孔により膀胱子宮窩腹膜下に血液が貯留したことが原因と考えられた難治性頻尿の1例を経験したので報告する．症例は55歳，未閉経，4妊3産で3回の帝王切開の既往がある．頻尿と腹痛のため抗菌薬による治療を受けたが改善なく，骨盤MRI検査を受けるも経過観察とされていた．症状出現の9か月後に当科を受診した．月経は不順で少量の褐色帯下が持続していた．経腟超音波検査では膀胱と子宮の間に囊胞性病変を認め，前医でのMRI画像では子宮体部下部の帝王切開瘢痕部が菲薄化し，膀胱頭側に長径6cmの液体貯留があり，この部分に接する膀胱壁が肥厚していた．病歴および画像所見より帝王切開瘢痕部に瘻孔が生じ，膀胱子宮窩腹膜下に血液が貯留した状態と診断した．年齢を考慮しリュープロレリンによる偽閉経療法を導入した．頻尿，腹痛は徐々に改善し，囊胞部分も縮小し帯下も消失した．
