症例

無症状で発見され，その後繰り返す胸腔子宮内膜症性気胸に対して両側付属器摘出術が奏効した1例

▶要旨

胸腔子宮内膜症は稀少部位子宮内膜症のrare siteに分類され，胸腔子宮内膜症性気胸（TEP）を引き起こすことがある．47歳の女性で，検診時に無症状の状態でTEPが発見され，その後胸腔鏡下手術およびレルゴリクスおよびジエノゲストを実施したが，TEPを繰り返すため，気胸予防を目的に腹腔鏡下両側付属器摘出術を実施した．左卵巣は4cm径に腫大し，病理診断は卵巣子宮内膜症性囊胞であった．術後1か月のFSHは43.2mIU/mL，E₂は5pg/mL以下で閉経状態にあり，術後5か月現在，気胸の再発はない．TEPで薬物療法に抵抗性を示す難治例に対して，両側付属器摘出術は有効な治療法と考えられた．
