浸潤性インプラントを認めたseromucinous borderline tumor（SMBT）ⅢB期の1例

▶要旨

症例は43歳，術前診断は進行卵巣癌の疑いで開腹術を施行した．腹腔内に淡血性腹水を認めた．両側卵巣は腫大し，不整な乳頭状腫瘤を無数に認めた．胃脾間膜に1cm大の結節を認めた．左付属器の術中迅速病理組織診断はborderline tumorであったが，悪性の可能性を考慮し，卵巣癌に準じてリンパ節郭清を含めたdebulking surgeryを施行した．肉眼的に残存病変なく，手術を終了した．摘出標本の病理組織診断はseromucinous borderline tumor（SMBT）で，大網と胃脾間膜に浸潤性インプラントと骨盤リンパ節転移を認めた．腹水細胞診は陽性であった．境界悪性卵巣腫瘍ⅢB期（pT3bN1aM0）と診断し，術後にパクリタキセル・カルボプラチン（TC）療法を6コース施行した．最終治療後3年Xか月が経過するが，再発は認めていない．浸潤性インプラントを有するSMBTの術後化学療法の有効性は確立されていないが，ガイドラインでは卵巣癌に準じた治療が勧められている．術後化学療法の有効性に関しては今後もSMBT症例の集積と検討が必要である．
