はじめに

　がん手術が安全に行われるためには，解剖学的知見と手術理論との間のギャップを埋める手術解剖学が必要である．近年，骨盤自律神経の解剖学的研究が進み，また未固定解剖体を用いた手術手技のシミュレーションによるトレーニングが子宮頸がん手術にも応用されている．しかし，解剖体における臓器間の位置関係は，臓器を牽引してカウンタートラクションにより臓器間の膜構造を緊張させて行う実際の手術とは異なる点に留意が必要である．また，実際の手術での出血リスクを解剖体で評価することも困難である．新しい手術手技を一般化するためには，術式の標準化，手術操作の定型化が必要である．

　本稿では，広汎子宮全摘出術における膀胱子宮靱帯後層（膀胱腟靱帯）の処理に焦点を絞り，実際の手術と未固定解剖体の検討に基づき子宮頸がんの手術解剖学の解説を試みる．
