全身性エリテマトーデスの増悪

遭遇しやすい典型ケース

　30歳台．初産婦．20歳台に全身性エリテマトーデス（SLE）と診断されていたが，転居を契機にフォローアップは自己中断されていた．定期的に発熱していたが，近医で処方された解熱鎮痛薬で対処していた．妊娠後は，妊婦健診を受けていた産婦人科医にSLEを診断されたことがあることは申告していたが経過観察とされ，妊婦健診を受けていた．妊娠26週，季節性インフルエンザに罹患し，1週間経過しても解熱せず，全身倦怠感が増悪したため健診施設を受診した．受診時，全身浮腫，高血圧，蛋白尿を認め，妊娠高血圧症候群（HDP）を疑われたため高次施設へ搬送された．搬送後，内科と共同で精査を行いHDPではなくSLEの増悪の可能性が強く疑われた．すぐにプレドニゾロン，免疫抑制薬の投与を開始したが，汎血球減少，腎機能低下，肺高血圧症など重度の臓器障害を認めたため，妊娠の継続は困難と判断され，搬送翌日に妊娠を終了した．
