子宮筋腫―進む病態理解と治療の実際

●薬物療法では，筋腫縮小と出血量減少を目的にエストロゲンとプロゲステロンの両者を低下させる方法と，出血量減少を目的としてプロゲステロン作用によって子宮内膜を菲薄化させる方法があり，前者は骨量減少や更年期症状が，後者は筋腫増大のリスクがある．

●外科的療法では，根治的な子宮全摘術を除き術後の再発が課題である．

●子宮筋腫の約70％を占めるMED12遺伝子変異陽性筋腫は，平滑筋細胞と線維芽細胞の比率が同程度で膠原線維が豊富な腫瘤である一方，MED12遺伝子変異陰性筋腫は平滑筋細胞が優位で線維芽細胞は少ない特徴を有する．
