腹腔鏡下に異なる治療を施行した非交通性副角子宮の2例

▶要旨

　単角子宮は全子宮奇形の0.3〜4.0％と稀な疾患で，非交通性副角子宮を伴うものはさらに稀有である．今回，われわれは異なる治療法で管理した非交通性副角子宮の2例を経験したので報告する．

　症例1 : 14歳，0妊．月経困難症のため当院に紹介となり，精査の結果，非交通性副角子宮を認めた．副角子宮と単角子宮は索状構造物を介するのみで，腹腔鏡下副角子宮切除術を施行した．

　症例2 : 35歳，0妊．下腹部痛のため近医を受診し，卵巣囊腫を指摘され，当院へ紹介となった．精査の結果，単角子宮左側に広基性に連続する非交通性副角子宮と左卵巣子宮内膜症性囊胞を認めた．早期の妊娠希望があり，副角子宮は切除せず，腹腔鏡下卵巣腫瘍摘出術を施行した．

　副角子宮切除術における手術の難易度は副角子宮と単角子宮の連結の程度に依存する．月経困難症の改善とともに，妊孕性を損なわない治療を選択する必要があり，個々の症例に応じた対応が必要である．
