肝臓癌治療成績と腫瘍マーカーの推移―1．内科的治療

〔SUMMARY〕　肝細胞癌患者の多くが，肝硬変を合併しているため，肝機能を温存しながら同時に局所根治性も期待できる経皮的局所療法が，わが国で開発され発展してきた．3cm以下3個以下という条件に限れば，切除に匹敵する生存率が報告されている．肝細胞癌の腫瘍マーカーとしてAFP，AFP-L3，PIVKA-IIの3種が測定可能であるが，その中でもAFP-L3は，局所療法後の画像で描出されない遺残癌の検出に威力を発揮した．3種の腫瘍マーカーいずれもが，腫瘍径や腫瘍数といった癌の進行度とは独立した予後因子であった．特にPIVKA-IIは，門脈腫瘍浸潤を予測するという独自の性質をもっていた．〔臨床検査　49：1225-1230，2005〕
