外リンパ瘻の診断マーカーとしてのCochlin-Tomoprotein(CTP)

1．外リンパ瘻とは

　「外リンパ瘻」は耳鼻咽喉科診療以外ではあまり知られていない病名で難聴，耳鳴り，めまい，平衡障害など様々な症状を呈する疾患である¹⁾．外リンパ瘻は外傷性難聴，内耳梅毒，ウイルス(ムンプスやVZV)性難聴，薬剤性難聴などと同様に発症原因に基づく診断名である．内耳疾患の病名で一般的によく知られているのは，突発性難聴，メニエール病などであるが，これらは症候学的診断名であり，特発性であることがその診断基準に明記されている．これらの症例の中に外リンパ瘻が少なからず含まれていることが以前より指摘されており本稿で述べるCTP検出法が確定診断に役立つことが期待されている．

　内・外リンパは内耳の中を満たす液体で，それぞれ内リンパ腔(図1の赤い部分)，外リンパ腔(図1の青い部分)に存在する．音波は空気の振動としてリンパへ伝わり蝸牛有毛細胞を刺激し音として知覚される．またリンパの流動は半規管を刺激し平衡機能を司っている．つまり内耳のリンパは，聴覚・平衡機能を司るために決定的に重要な働きをしている．外リンパ瘻は，外リンパが内耳から中耳へ漏出することによって，内耳の生理機能が傷害される疾患である．漏出部位は，前庭窓，蝸牛窓と呼ばれる内耳窓や内耳のmicro-fissureなどである．
