1．はじめに

　悪性腫瘍細胞を呼ぶ言葉によく異型細胞が用いられる．これは癌細胞も含めるが広く癌の疑いが濃いものを意味し，核クロマチン増量，核・細胞質比の増加，細胞質の塩基好性，核小体肥大と増加，異常核分裂，細胞の形と大小の不同などが細胞異型度の表示に用いられる．構造異型は正常組織との隔たりの大きさで表現される．超微構造について癌細胞の特異構造はなく，光顕的異型像に加えて細胞質小器官の発達の乏しいこと，形質の脱落の見られることである．一方，形質の保持は腫瘍分類の標識となっている．明らかな組織学的特長は浸潤性増殖の開始であり，非連続的増殖や転移像は決定的である．1950年代この事実を認識したうえであえて細胞診を始めることは無謀と思われて不思議はなかった．

　米国留学後病理医として最初の出発が東京鉄道病院という一般病院であったのは幸いであった．国鉄のセンター病院で腫瘍の病理検査は多いものであったこと，マイアミ大学ではHopman先生からはじめてパパニコロウ染色標本を見させられ(1957)，赤，橙　黄，青緑，に染まる多彩な扁平上皮細胞に混じって大きな癌細胞を見出したとき，まさに万華鏡を覗いたような強烈な印象であったことは私を細胞診導入に駆り立てることになった．就職早々臨床検査科の新設を任せられており，後にリンパ球培養核型分析も取り入れて当時としては先端を行く病理検査室であったと思っている．先にマイアミ大学から帰国早々の河合忠先生が臨床化学，血液，特に血漿蛋白免疫電気泳動室を開設し¹⁾，臨床病理科の原型を発足出来たと考えている．“めくら蛇におじず”の意気で日本病理学会総会に“喀痰による肺癌の診断”を発表し病理学会の諸先輩から，ばらばらの細胞で癌の病理診断をするという無謀なことと，つよいお叱りを受けたことは今も記憶にあたらしい²⁾．

　“To be, or not to be：that is the question”(Shakespeare)．有名な言葉はまさにそのときの心境であった．
