β-トレース蛋白と髄液漏

1．はじめに

　近年CTやMRIの出現により脳脊髄液(CSF)検査は著しく実施頻度が減少し，現在では外観観察，比重，細胞数，好中球/リンパ球(N/L)比，総蛋白量，ブドウ糖，クロール，IgG定量などが日常検査として行われ，髄膜炎，脳炎，Guillain-Barré症候群，多発性硬化症など炎症疾患の鑑別，補助診断に利用されている．

　鼻内髄液漏は，交通事故，墜落などの頭部外傷，副鼻腔手術，内視鏡検査などの損傷により，頭蓋底骨折により髄液が鼻腔に漏出する病態である．髄液漏の存在を確認することは，髄膜炎，脳炎などの予防に極めて重要である．

　これまで簡易検査としてテストテープによるブドウ糖検査，髄液特異性の高いβ₂-トランスフェリンの検出，より精密な診断法としては¹¹¹In-DTPAによるシンチグラフィー，鼻液の放射活性の検出が行われているが，血液，涙液の混在による特異性や検査の煩雑性などに課題が残されていた．

　これまで利用されてきたβ₂-トランスフェリンは，確かに脳脊髄液に特異的に存在することから，髄液漏の補助診断として有用である．しかし，免疫固定法やウエスタンブロットで検査法が煩雑な欠点がある．Papadeaら¹⁾の報告では必要検体量が2mlを要するとされ，Mecoら²⁾によれば，オーストラリアでの検査費用は50ドル(約4,000円)と高価であることが指摘されている．

　最近，血清と比べ脳脊髄液中に高濃度に存在することに着目して，特異抗体を作製，免疫学的測定法によるβ-トレース蛋白(BTP)による髄液漏の検査が注目されている．
