センチネルリンパ節生検

　センチネルリンパ節(SLN)生検は臨床的にリンパ節転移の見られない早期乳癌の手術療法において腋窩リンパ節郭清を省略できるかどうかをみる方法として世界的に広く行われている．SLNの同定にはアイソトープ(RI)法，色素法またはRI法と色素法の併用法が多く行われている．わが国ではほとんどの場合，SLN転移の有無は術中の病理診断か細胞診で検索され，転移陰性ならばリンパ節郭清を省略し，転移陽性ならば郭清を追加する．問題点としてSLN同定不能例やSLN転移陰性で非SLN転移陽性例(偽陰性例)がありえること，術中病理診断における転移の見落とし(術中診断の偽陰性)，0.2mm径以下の転移巣や免疫染色の意義が未確定であること，などが挙げられる．〔臨床検査　51：55-60，2007〕
