乳腺細胞診の新報告様式

はじめに

　細胞診においては1988年に婦人科細胞診でベセスダシステムが発表されてパパニコロウのクラス分類の問題点が指摘されて以来，従来から広く用いられていたパパニコロウ分類に代えて記述的な報告様式を用いるという傾向が強まってきた．このような流れのなかで，乳腺細胞診に関しても1990年代後半には欧米において新しい報告様式が提唱されるようになった^1,2)．わが国においても，土屋，垣花ら³⁾を中心に日本臨床細胞学会や日本乳癌学会においてパパニコロウのクラス分類に代わる乳腺細胞診の新しい報告様式が検討され，その結果2003年に“乳腺における細胞診および針生検の報告様式ガイドライン”⁴⁾が発行された．このガイドラインでは細胞診，針生検ともに同様の報告様式を使用することを提唱している．

　本稿では乳腺細胞診のガイドラインに示された新報告様式の概要を解説するとともに筆者らの診断成績などを参照しながら新報告様式の特色について紹介する．
