はじめに

　肝炎ウイルスには主に経口感染するA型肝炎ウイルス(hepatitis A virus；HAV)，E型肝炎ウイルス(hepatitis E virus；HEV)と主に血液を介して感染するB型肝炎ウイルス(hepatitis B virus；HBV)，C型肝炎ウイルス(hepatitis C virus；HCV)，D型肝炎ウイルス(hepatitis D virus；HDV)がある．A型肝炎は全世界に分布し，経口水系感染であることから衛生環境の指標とされている．E型肝炎も人畜共通感染症であり，HAV同様に衛生環境の悪い東南アジア，中央アジア，アフリカ，南米などの発展途上国で流行が報告されている．最近，国内での散発性急性E型肝炎の報告が相次いでいる．しかし，HAV，HEV感染ともに劇症化する稀なケースを除けば，多くは一過性の感染で慢性化することはない．一方，主に血液を介して感染するHBV，HCV，HDVは慢性化し，慢性肝炎，肝硬変をへて肝細胞癌に至るので，世界中で大きな問題となっており，ウイルス駆除を目的とした治療が積極的に行われている．

　ここでは，慢性肝炎の主要原因であるB型・C型肝炎ウイルスの臨床的意義，およびその診断には欠かすことのできない血清学的検査に加えて遺伝子検査の使い方について述べる．
