はじめに

　Methicillin-resistant Staphylococcus aureus(MRSA)は，1961年にメチシリン(methicillin)に耐性を示す黄色ブドウ球菌として最初に報告された．1980年代には日本でも全国で報告されるようになり，現在では臨床で分離される黄色ブドウ球菌の約60～70%がMRSAであると報告されている¹⁾．検査室におけるMRSAの検出は，一般的にClinical and Laboratory Standard Institute(CLSI)の基準に則って，2%NaCl添加Muller-Hinton brothによる微量液体希釈法で，オキサシリン(oxacillin：MPIPC)を4μg/ml以上のMIC値を示す場合，または2%NaCl添加Muller-Hinton agarを用いたディスク拡散法で，1μgを含むKBディスクの発育阻止円が10mm以下の場合に，MRSAと判定している．PCR法をはじめとする遺伝子検査は，一般の病院検査室では，種々の問題からあまり実施されていないが，感染症の迅速診断法として今後の発展が期待でき，また，MRSAの病原性の研究や，年々重要さを増す病院感染における感染経路の特定などにも有用である．個々の菌の薬剤耐性遺伝子や毒素遺伝子など病原遺伝子保有などの迅速診断は感染症の早期治療に役立ち，また，遺伝情報に基づいたMRSAの疫学調査を行うことができる．特に近年，市中型MRSA(community associated MRSA；CA-MRSA)が増加し²⁾，これらが病院型MRSA(healthcare associated MRSA；HA-MRSA)とは遺伝的に異なっていることが報告されるようになってから，その違いを遺伝学的に解析することがよく行われるようになってきている．
