1) 膠原病，自己免疫疾患

はじめに

　自己免疫疾患は人口の約2～5%がなんらかの疾患に罹患しているとされ，決して稀な疾患ではない．自己免疫疾患の発症には遺伝素因の関連が知られている．その根拠としては多発家族が存在すること，関節リウマチ(rheumatoid arthritis；RA)，全身性エリテマトーデス(systemic lupus erythematosus；SLE)，多発性硬化症(multiple sclerosis；MS)，インスリン依存性糖尿病において，二卵性双生児で発症一致率は約5%に対し，一卵性双生児では発症一致率が約25～50%と高いこと，遺伝的に規定された自己免疫疾患モデル動物が存在することが挙げられる．これまで自己免疫疾患の疾患感受性遺伝子は数多く同定されている．しかし，診断や予後の推定における有用性が示され臨床の場に導入されているものはヒト白血球抗原(human leukocyte antigen；HLA)のみである．近年，RAでは薬物代謝酵素遺伝子であるmethylenetetrahydrofolate reductaseの一塩基多型C677Tがmethotrexateの副作用発現の頻度と相関することが示され，今後の臨床への導入が期待されている．

　本稿では臨床検査として自己免疫疾患の診療に有用な遺伝子であるHLAに焦点を当てて概説する．
