子宮頸癌の予防・早期発見のための検査手順(トリアージ：triage)

　子宮頸癌は検診によって前癌状態を検出し，早期発見することによって，予防が可能な癌腫である．現在，わが国で行われている前癌病変検出のための検査手順とその後の取り扱い(トリアージ：triage)や予防的治療法について概括した．欧米では既にHPV-DNA検査が子宮頸癌検診に導入され，成果を上げている．HPV-DNA検査を日本の検診システムに導入するために，これに対応した細胞診の報告様式であるベセスダシステムや，細胞診検査後にHPV-DNA検査を追加可能な液状処理細胞診を同時に導入することが望ましい．HPV-DNA検査を補助診断法として用いると，細胞診で評価が困難なASC-USについても高度病変存在のリスク評価が可能となる．一次スクリーニングにおいて細胞診とHPV-DNA検査を併用すると，子宮頸部高度病変に対する感度が向上し，特に陰性適中率が向上するため検診間隔を延長できることが示されている．
