超音波検査による甲状腺結節の新しいクラス分類とその妥当性

　甲状腺結節の超音波診断に新しいクラス分類を導入し，穿刺吸引細胞診と病理組織検査を対比させてその妥当性を検討した．結節を超音波所見により，USC1～5に分類し，さらにUSC2～5の間は，クラスの両方にまたがる所見をもつ結節については，0.5刻みで中間のクラスを設け，USC3.5以上が悪性，3はボーダーライン，2.5以下は良性と判定した．検討対象の結節1,145個(900症例)のうち，超音波検査で悪性と判定された結節は233個であった．このうち179個(76.8%)が細胞診でも悪性と診断された．142個が外科的に切除され，136個(95.8%)が病理組織検査で甲状腺癌と診断された．超音波検査で良性と判定された結節710個のうち，683個(96.1%)は細胞診でも良性であった．われわれの新しい超音波検査診断のクラス分類は細胞診および病理組織診ともよく対応しており，日常の臨床検査として普遍性のある優れた方法と言える．
