ヘパリン起因性血小板減少症(HIT)とその抗体検査

　ヘパリン起因性血小板減少症(HIT)は低分子ヘパリンを含めたヘパリンの重大な副作用である．HITの病因として，ヘパリンと血小板第4因子の複合体を抗原として誘発される自己抗体(HIT抗体)によるとされている．HIT抗体の検出には，酵素免疫測定法(ELISA)が本邦を含めて，広く普及しつつある．特にELISAの測定技術の改善により，日常的に安定した結果が得られ，その測定結果からHITにしばしば合併する血栓症の予測も可能となった．今回は，ELISAによるHIT抗体の意義を中心に概説する．
