アテローム血栓性脳梗塞における血中ADAMTS13(VWF-cleaving protease)濃度の測定意義

　慢性期アテローム血栓性脳梗塞(ATBI)50症例を対象として血漿ADAMTS13濃度をELISAにて測定した．アテローム硬化度別に4群に大別し(Ⅰ～Ⅳ群の順にrisk大)比較検討した．血中ADAMTS13はアテローム硬化度のより強いⅢ～Ⅳ群において有意に低値を示し，さらにⅢ群よりⅣ群において有意に低値であり，対応基質である因子(VWF)抗原量と有意に逆相関した．以上の結果よりATBIを代表とするアテローム血栓性病態においてはアテローム硬化度に応じて血中ADAMTS13は減少し，VWFの増加を介してアテローム硬化の形成・増悪進展因子として作用しているとともに，血中ADAMTS13濃度はアテローム硬化度を反映する分子マーカーの1つになり得ることが強く示唆された．
