前立腺癌の診療に関するノモグラム

1．前立腺癌診療におけるノモグラムの利用
ノモグラムとは，一般に，ある特定の関数の計算を行う目的で作成された計算図表を指す．医学臨床においては，特定のアウトカムの確率を予測するための計算手段として，ノモグラムが作成され用いられている．前立腺癌患者の病態や病状には大きな差があり，予測される予後や選択可能な治療選択肢も多彩である．そこで，前立腺癌の診療において，種々のパラメータを用いて，リスクを評価したり予後を予測する手法として，多くのノモグラムが開発発表されてきた．
パラメータとして統計学的に有意な予後推測因子が選定され，各々のパラメータに重み付けをして点数化し，総計を求めて全体の推定値を計算する(図)1)．具体的には，血清前立腺特異抗原(prostate specific antigen；PSA)の値，臨床病期，組織学的悪性度(主にグリソン・スコア)などがパラメータとして用いられる．また，推定するアウトカムとしては，生検陽性率，手術摘出標本の病理学的病期，再発率，生存率など，様々なものがある(表1)．一般に，ノモグラムによるアウトカム予測は，専門医の判断や単純なリスク分類よりも優れているとされている2)．
