自発脳磁図とその異常

 現在,臨床検査としての自発脳磁図の測定は,主に脳神経外科領域における術前・術後の評価として実地臨床において有効に用いられている.器質的脳病変を有する患者の徐波活動や,難治性てんかん患者の発作間歇期の棘波や鋭波などの突発性異常波について,脳磁図を用いてその波形の観察や脳内での発生源推定を行うことで,脳外科手術の成績向上に大きく寄与している.また,最近では統合失調症や認知症などの精神神経疾患において脳の局在異常を同定し,その病態解明に役立てる研究が相次いで報告されている.器質的脳病変に関する内容は前稿「脳磁図による脳病変の機能診断」に,てんかん領域に関する内容は次稿「てんかんスパイクの磁場源解析(成人)」に譲るとして,本稿では,このような自発脳磁図の特徴と,主に精神科領域への臨床応用として,統合失調症と認知症を中心に概説した.
