Helicobacter pylori除菌による胃癌予防

1．はじめに

　Helicobacter pylori(H. pylori)の出現以来，わが国の国民病とされてきた胃癌の概念が大きく変化している．H. pylori感染が胃癌の発症に深く関与していることが明らかになり¹⁾，さらに最近，わが国において早期胃癌に対する内視鏡的治療後のH. pylori除菌による異時性胃癌抑制効果を検討した大規模な無作為化比較試験(randomized controlled trial；RCT)の結果²⁾がLancet誌に報告され，除菌による胃癌予防が現実的なものとなっている．胃癌の予防という観点では，一般的には食生活改善による予防法が注目されているが，本稿では，H. pyloriと胃癌との関連についての現状を概説し，除菌による胃癌予防の可能性を考察する．
