2009年までにWHO(World Health Organization)のIARC(International Agency for Research on Cancer)は,飲酒が原因で発癌する臓器は口腔,咽頭,喉頭,食道,肝臓,結腸・直腸と女性の乳房であり,アルコール飲料,飲料中のエタノール,飲酒と関連して発生するアセトアルデヒドの3つを,発癌物質と結論づけた(表)1).2005年の厚生労働省多目的コホート研究(Japan Public Health Center-based prospective Study;JPHC Study)は,飲酒関連癌のリスクは1日1合未満相当(以下日本酒換算,ビール500mlに相当)の習慣飲酒者では,ときどき飲酒する人の2.3倍であり,男性の癌死亡の13%が1日2合の飲酒に起因すると概算した2).
