B型肝炎

はじめに

　B型C型慢性肝炎のガイドラインは毎年更新されている．これは，ひとえに熊田博光班長を中心とする「厚生労働省厚生科学研究費肝炎等克服緊急対策研究事業(肝炎分野)ウイルス性肝炎における最新の治療法の標準化を目指す研究班」の尽力によるものが大きい．同時にこれらのウイルス肝炎の診断もしくは治療法が毎年何らかの進歩をなしていることを意味する．

　2011年5月からB型肝炎ウイルス(hepatitis B virus；HBV)ゲノタイプの測定が可能となった．また，9月にはB型慢性肝炎に対してpeginterferon α-2a(商品名：ペガシス^®)の48週間投与が認可された．さらには，以前は治癒したと考えられていたB型肝炎既感染者から免疫抑制療法もしくは化学療法後にHBVが再活性化するde novo B型肝炎が新聞の記事を賑わせた．

　本稿では，このようなB型肝炎に関する診断法・治療法の変遷を振りかえりながら，現行のガイドラインに基づいた診断法・治療法につき述べたいと思う．
